The only way to get *all* context switching is IMO to add a 
RUBY_EVENT_CONTEXT_SWITCH event on which event hooks can subscribe. The patch 
I submitted to Charlie does not catch some cases

Another thing that would be nice to add is RUBY_EVENT_YIELD and 
RUBY_EVENT_YIELD_RETURN events. That would allow profilers to account the 
time spent in the yield in the caller (the method which created the block), 
and not the iteration method.
