bigdecimal/math.rb の BigMath は、利用者が include してつかうことを前
提としていますが、おかしなことで、それなら最初からそのようにしておけば
よいはずです。本当は、BigMath のメソッドをモジュール関数にしておけば、
Math 同様、include してもしなくても、利用者の判断でつかい分けてきます。
BigMath のメソッドはモジュール関数にしたらいいと思います。

bigdecimal/util.rb に Integer#to_d がありません。

bigdecimal/util.rb の Float#to_d は、BigDecimal(self.to_s) しているだ
けです。わざと甘くしている可能性もありそうですが、もっとちゃんとするこ
ともできると思います。修正すると、うちでは Math::PI.to_d.to_f ==
Math::PI が成り立ちました。
