これはそもそもSetがHashベースで実装されているのに、mutableな要
素を使って(さらにそれを変更して)いることに問題があります。
これが起きるのはdupだけではないので、initialize_copyでrehashす
るだけでは充分ではないはずです。

  $ ruby -rset -e 'a=[];s=Set[a];p s.include?(a);a<<1;p s.include?(a)'
  true
  false

また、Hash#hash自体も自己参照している場合エラー(あるいはSEGV)に
なります。

  $ ruby -e 'h={};h[h]=1;p h.hash'
  -e:1:in `hash': stack level too deep (SystemStackError)
          from -e:1:in `hash'
          from -e:1:in `hash'
          from -e:1:in `hash'
          from -e:1:in `hash'
