I wouldn't want a program to write a BOM at the start of a file
without being asked, in particular not for UTF-8 (more likely for UTF-16).

So we could do something like

open("file.txt", "w:utf-8::with-bom") do |file| ... end

Somewhat similar choices may be needed for input:
- accept a bom and remove if present
- accept a bom and keep as part of the datastream
- throw an error if there is a bom

All three scenarios have some good usages.

For UTF-16, it gets quite a bit more complicated.
There's not only the BOM, but also UTF-16/UTF-16BE/UTF-16LE,
and two different endiannesses. Also, internally, we may
have "UTF-16, machine endianness" as an encoding.
