How about the following?

$ cd ext/tcltklib
$ CONFIGURE_ARGS='--with-tcl-include=/usr/local/include/tcl8.3/ --with-tcllib=tcl8.3 --with-tklib=tk8.3 --enable-tcltk_stubs' ruby extconf.rb
