OK, more information after some debugging. I did find that it's important to
specify the cursor's "hotspot" coordinates, even though they currently have
defaults of -1. Windows will not create the cursor otherwise. Also, the GIF
image size must be 32x32 pixels because this is the standard cursor size on
Windows.
