Add /D "NT" and /D "IMPORT".

If you don't hate commandline operation, the most easy way
is to write and to use extconf.rb.

Example:

  C:\somewhere>copy con extconf.rb
  #!ruby
  require 'mkmf'
  create_makefile('tstruby')
  ^Z
  
  C:\somewhere>ruby extconf.rb
  creating Makefile
  
  C:\somewhere>nmake
  ...
  
  C:\somewhere>nmake install
