Subject: ANN: REXML 2.1.2

This release is a bug fix release.

2.1.0 was missing output.rb from the distribution, since I had
forgotten to add it to the repository.  In 2.1.1, Document failed to
require output.rb, forcing you to do the require in your code and
subsequently breaking older apps.  Also, there was a bug in the code
that read non-UTF-8 streams from files, and this has been fixed.
