In order to avoid the hash vs. hash semantics why not just add a
different constructor?

OpenStruct::new :a => 42
behaves as before
OpenStruct::defaulted :a => 42
sets the hash :a => 42 as default value.
Naming has never been one of my many ;) talents, but I guess you got the idea.
