Let's check your http.conf file.

Do you have the following lines somewhere in http.conf?

  # for Apache::ERubyRun
  RubyRequire apache/eruby-run


 # handle *.rhtml as eruby files.
  <Files *.rhtml>
    SetHandler ruby-object
    RubyHandler Apache::ERubyRun.instance
  </Files>

It looks like it's dying trying to open the file. I'd also check the
permissions of the file.  Is it owned by your apache account (www-data
on Debian)?  Or readable by the www-data group?  Is foo.rhtml in a 
directory readable by apache?
