When Sun was trying to address the namespace conflict problem in Java they used domain names (com.sun.???) and wrote it into their "standard".  There may be a day when Ruby is installed on lots of machines and the need for library management becomes something that needs to be automatic and not manual.  You may already be looking toward this and have ideas in mind...but perhaps a naming of extensions in that way would be positive (i.e.. require "com/domain/app/foo").  I realize its just convention, but if we all use it it removes some of the problems of collision.
