 You have an example in comp.lang.ruby

 Search the thread  "Retrieve email attachments using Net::IMAP" in google

 Basically when you have a message id, you can do

  image.fetch(message_id, "BODY[1]")
  image.fetch(message_id, "BODY[2]")
