And you missed useful macros defined in rubyio.h? :-P

    OpenFile *fptr;
    FILE *fp;

    GetOpenFile(io, fptr);
    fp = GetReadFile(fptr);


....well it's pretty long and redundant, but somewhat `official' and
usable sound way.
