True 'nuf. Planting the <a name=>'s is easy. The problem comes in
generating the sorted index itself (some of the index entries are
pretty horrific, because of all the font changes and other fooling
around). A pretty horrendous example is the following:

\index{\protect\Ikey{<<\$ variables>>}@\texttt{\$} variables!ZZZZZ@\emph{English names}}

which puts the italic 'English names' entry at the end of the list of
$ variables (you can see it in all it's glory on page 537).

However... we might be able to use TeX's Makeindex to postprocess this 
for us. I'll look in to this sometime next week.
