There's no DTD, but it shouldn't be hard to make one up automatically: 
all that stuff was generated automatically, so the structure is fairly 
mechanical.

The XML is going to be extended soon as I put the rest of the book 
online. So far the changes are (I think) compatible with the existing 
reference.

(If anyone _does_ create a DTD, I'd love a copy.)
