結局 {cygwin,mingw32}-ruby-1.6.dll にしました.

それはそれとして, dlltool だと IMPORTS というのがあって,
これで同じように module-name を指定して dummy DLL が作れました.

bintuils/dlltool.c の冒頭:
> IMPORTS  (  (   <internal-name> =   <module-name> . <integer> )
>           | ( [ <internal-name> = ] <module-name> . <external-name> )) *
> Declares that <external-name> or the exported function whoes ordinal number
> is <integer> is to be imported from the file <module-name>.  If
> <internal-name> is specified then this is the name that the imported
> function will be refered to in the body of the DLL.
